Maternity, Paternity and Adoption

The online form should be completed by employees. Printable versions can be obtained within the other forms section, for managers to obtain on behalf of employees with no access to a work computer.

  • Please refer to Family Leave Policy before applying 
  • Please refer to Flexible Working Policy if required.
  • Submit Maternity Leave requests no later than the 15th week before the expected week of childbirth. Please upload a copy of your MATB1 to support your application.
  • Submit Paternity Leave requests 4 weeks before the expected week of childbirth. Please upload a copy of your MATB1 or birth certificate to support your application.
  • Submit Adoption Leave within 7 days after employee is notified that they have been matched with a child. Please upload a copy of your Matching Certificate from the approved adoption agency to support your application.

Keeping In Touch Days

Line Managers should agree Keeping In Touch Days (KIT days) with employee in advance. This form should then be submitted online by Line Manager after KIT days have been worked.

Change of Details - Line Manager

The changes listed below should be submitted online by line managers:

  • Acting Up (Maximum period is 6 weeks)
  • Change in Status/Gained Permanency
  • Temporary Increase in Hours
  • Change in Work Location/Transfer
  • End of temporary change
  • Termination

For employees with access to Employee Self Service (ESS), changes to personal details should be submitted through iTrent.  A printable version can be obtained within the other forms section, for managers to obtain on behalf of employees with no access to a work computer.
