District Nursing
The District Nursing service provides care to house-bound patients, and also to patients in settings such as care homes. It is committed to delivering high quality and compassionate care.
All patients have a named nurse responsible for assessing, planning, delivering and evaluating care in partnership with patients and their carers.
District nursing teams are aligned to GP practices, and each GP practice has a named district nurse that provides a leadership role in terms of communication and engagement.
The District Nursing service works with all health services and integrated partners such as voluntary organisations.
Planned Care
District nurses are involved in the delivery of planned care to individuals with both complex and non-complex health problems. District nursing interventions may be short term or longer, based on the need of the individual patient. Planning for discharge from the district nurse caseload is considered at the beginning of the patient’s period of care.
Unplanned Care
In addition to planned care, district nurses often undertake unplanned episodes of care, if the patient’s condition dictates this, with the aim of preventing hospital admission and the rehabilitation of individual patients.

The Community Diabetes Specialist Nursing Service provides specialist care and advice to people living with diabetes who have been referred to the service. People living with diabetes may be reviewed at home, in a homely setting or in another setting suitable to or defined by the person’s needs.
Patients are allocated to a member of the Community Diabetes Specialist Nursing Service, who will then be responsible and accountable for their care provision. This specialist nurse will be responsible for assessing, planning, delivering and evaluating care in partnership with patients, their families and carers.
On-going care may be provided by the nursing service, or a Community Adult Nurse under Community Diabetes Specialist Nurse supervision.
The service also offers structured group education for individuals who are newly diagnosed with Type 2 diabetes.
