Declaration

I understand that:

  • The information I provide must be complete and correct, if it is not, I may be prosecuted.
  • the Council will withdraw an offer of a place if it was obtained through a fraudulent or intentionally misleading application.
  • that my child’s school place can be withdrawn even if he/she has started at the school if the place was obtained through a fraudulent or intentionally misleading application
  • that I must promptly tell the Council’s Shared Services Education Team (sharedservices.education@eastdunbarton.gov.uk) of any change of address, for either or both myself and the child. Email contact details will be provided to you on the acknowledgment email issued in respect of this application

I am aware of my right to a Gaelic medium education. Furthermore, I have been advised that I may elect to defer my child’s entry to Primary 1.

Important Information

You can check your catchment school on the school catchment webpage. 

Please note you will be asked to provide the following documents as proof of residency at the end of this form:

  • utility bill, with your current address, dated within last 3 months.   A utility bill can be from your gas/electricity/broadband/landline phone provider or a TV Licence. 
  • current council tax bill
  • child's birth certificate
  • proof of guardianship (if applicable)
  • Child benefit or bank statement showing Child benefit is paid to you

We will accept a screen shot of online accounts/ bill if you do not receive paper bills this this should show your name and address.  We will also accept a clear photograph of documents.
