Please complete the following Alteration Request Form if:

  • your child is returning for their 4 year old place in August and you wish to alter their pattern of attendance
  • your child is currently in nursery and you wish to alter their pattern of attendance
  • you would like to inform EDC of a change of details i.e. change of address, change of child's name
  • you wish to submit evidence or a document required for your child's place.
Declaration

I agree that the information I give on this form is complete and correct.
I understand if I give wrong or incomplete information I may be prosecuted.
I understand that the Council will withdraw an offer of a place if it was obtained through a fraudulent or intentionally misleading application.
I understand that my child’s early learning and childcare place can be withdrawn even if he/she has started at the centre if the place was obtained through a fraudulent or intentionally misleading application.
I understand that I must promptly advise the Council of any change of address details after the submission of my child’s application either by emailing the relevant early years centre or earlyyears@eastdunbarton.gov.uk.
I understand that places are allocated in accordance with the East Dunbartonshire Council Admission Policy and there is no guarantee that I will receive the session or centre requested.
