1 Start 2 Page 2 3 Complete Declaration The Care of Gardens service is the provision of garden maintenance which runs from April to October each year, whereby the Council will maintain the garden of East Dunbartonshire residents who meet the qualifying criteria. There is a charge for this service. To qualify for the Care of Gardens service, applicants must live in East Dunbartonshire and: Be part of a household where all members are aged 75 years or over OR Be part of a household where none of the adults (aged 16-74) are physically able to maintain the garden due to a diagnosed medical condition. Please note: the application process will require details of the medical conditions of those household members to ensure this criteria is met. Declaration Before proceeding with this Care of Gardens application, you must agree to the following declarations:- I have read and understood the qualifying criteria for this service. I confirm I am entitled to this service I agree the information on this form is complete and correct. I am aware I must immediately report any change in my circumstances which might affect my eligibility for this service, to the Shared Services Care of Gardens team. For example: my medical condition(s) improve or a physically able 16-74 year old capable of maintaining the garden becomes part of the household I understand if I give wrong or incomplete information, this may invalidate my application or the service provided could be withdrawn.
