Transition from Children’s Services to Adult Services

person celebrating their birthday by having birthday cakeThe booklet “Transition from Children’s Services to Adult Services – the Process” has been written to assist young people with additional support needs and their carers to understand the process which takes place to determine if the young person is entitled to formal paid services when they leave school or other Children’s services.  Services may include respite services and day services.  The procedure for calculating the level of support is explained; i.e. how many days the young person is able to access at a Day Service, or, what a person’s budget might be.

birthday partyWithin the booklet there is guidance in relation to Self-Directed Support (Direct Payments and other SDS options) and Adults with Incapacity (Welfare and Financial Guardianship).  There are also links to EDC policies. 

The booklet has been reviewed by professionals from Children’s and Adult Social work Services, from Education, and by members of the Learning Disability Strategy Group.  The booklet has also been shared with Parent and Carers groups during the consultation.   

Information on the site will be updated as changes to legislation and procedures occur, such as the likely implementation of the Disabled Children and Young People (Transition to Adulthood) (Scotland) Bill which would place new responsibilities on local authorities in relation to transitions. 

The booklet will form one of a collection of three booklets, the first providing general information about social and other supports available to people with learning disabilities and /or autism and the other focussing on the different transitions children experience as they go through the Education system.
